NHS England pays for all expatriates medical care and medication.
To first come to Spain as a visitor be it holiday or to stay you should have a GHIC (UK Global Health Insurance Card). If you wish to stay longer that 90 days you have to register for a residency card from Spain, this is now a TIE (Card Identification Foreigner). If not retired the rules may be different. I am a pensioner and have lived here now for 26 years and am a permanent resident, also married to a native am entitled to free health care as her dependant but Sain would sooner make Britain pay and why not I paid all my N.I. contributions in the UK. Having said that if you paid National Insurance in the UK under an EU rule, The country where you paid your N.I. contributions has to reimburse the EU state you settle in. NHS England said that 80% of the NHS budget goes to paying for the health care for expatriates living in other EU states.
Note: before 01.01. 2021 the rules are different for residents but there are similar agreements for those wishing to emigrate to Spain after that date and the Gov.UK web page has full information and you can sign up for permanent up dates, which I strongly recommend.
